A Few Trophy Based Questions

Hello, I got Soul Ignition last Friday, and I have not once been able to use a counter. I don't even know how to use one. The only reason why I know there is one is because the Hisagi trophy requires you to use a counter attack 100 times.

I also am wondering what I am suppose to do on the AA, Mayuri, and Ikaku trophy, as the translators didn't really help me understand. Can anyone help me?
